Nov 10-15 & 17, 2014 Supporting Diabetes Awareness Week

12th Annual Diabetes Awareness Week

"Health Mind, Healthy Body"

November 10th - November 15th, 17th

This November, in recognition of National Diabetes Awareness Month, Near North will host its 12th Annual Diabetes Awareness Week at our 9 locations throughout Chicago.  Participants will receive free and confidential health screenings for Blood Sugar, Blood Pressure, and Body Mass Index. Educational workshops, diabetic- friendly cooking demonstrations, and giveaways will also be provided.

Since 1998, Near North's Diabetes Collaborative Program has been committed to raising awareness about the prevention and treatment of diabetes and diabetes-related illnesses among low-income individuals living in Chicago, especially those at-risk in African-American and Latino communities. The Diabetes Collaborative is made up of a team of national and local healthcare partners—including Near North and Northwestern Memorial Hospital—who are dedicated to increasing access to diabetes care and decreasing poor health outcomes.
