Be sure to get your appointment BEFORE the March 31st deadline, to be sure you do not get hit with the penalty. Call or stop by to get signed up! Bring with you all the following paperwork:
- Social Security Numbers (SS cards or document numbers for legal immigrants)
- Proof of Residency (Drivers license or utility bill with your address)
- Employer and income information for every member of your household who needs coverage (for example, from pay stubs or W-2 forms-Wage and Tax Statements)
- Policy numbers for any current health insurance plans covering members of your household
