Auburn Gresham needs your help to improve the health of the community!
Our goal is to upgrade access to preventative health care and education by recruiting health service providers, expanding the offering of health fairs and increasing the awareness of nearby health resources.
Please take a moment to complete a 10-question health survey. Your responses will help us understand the levels of health knowledge and awareness in our community. The surveys are anonymous and will be used to improve health education and services.
